How to reserve a first appointment.

How do I book a first appointment?

Visit /book/, pick an open time on the calendar, and pay the $50 deposit to reserve. You then complete a pre-visit history through Spruce before the appointment.

How much do I pay today?

$50. That reserves your first appointment with Dr. D. The deposit applies toward the $249 start of care if care begins after the appointment.

How long is the first appointment?

The calendar reserves a 90-minute slot. The actual visit length depends on what the appointment needs; some first visits finish closer to 30 minutes, and some take most of the window. Dr. D listens, asks real questions, and decides next steps with you.

How is the first appointment delivered?

NorCal ADHD is telehealth-first. The first appointment runs on a secure call through Spruce.

What if I need to cancel or reschedule?

Cancel or reschedule at least 48 hours before the appointment to recover the deposit. Cancellation terms are shown again during booking.

Out-of-network, private-pay, superbills provided.

What does it cost if care begins?

$249 total start of care. That includes the first appointment, activation, and the first 4 weeks of care. Your $50 deposit applies toward the $249.

What does ongoing care cost?

$99 every 4 weeks. Care is charged by visit cadence (28 days), not by calendar month, so charge dates shift forward by about a month per year.

Do you bill insurance?

No. NorCal ADHD does not bill insurance directly. Patients pay the practice directly, and we provide superbills for possible out-of-network reimbursement.

Why do I need insurance if you do not bill it?

ADHD care can involve labs, primary care coordination, pharmacy requirements, medication authorizations, and other medical needs outside of NorCal ADHD. Insurance keeps that broader care available.

Will my insurance reimburse me?

Reimbursement depends on your plan. NorCal ADHD provides itemized superbills you can submit. We cannot guarantee reimbursement.

What if care does not begin after the first appointment?

The $199 after-visit charge is not made. The $50 deposit is non-refundable except when cancelled at least 48 hours before the appointment.

Spruce, follow-up, pharmacy, and how the practice stays involved.

What is Spruce?

Spruce is the HIPAA-secure messaging app NorCal ADHD uses for all care between visits. You message the practice 24/7; the team routes to Dr. D when his attention is needed. NorCal ADHD is not an emergency service.

How fast does the practice respond?

Guaranteed reply within 24 hours, every day. Real-time chat is not promised.

Does Dr. D reach out, or only respond?

Dr. D messages first after care begins. Proactive follow-up is part of how the practice runs, not an upsell.

Can I get a follow-up appointment between scheduled visits?

If something needs attention before the next visit, Dr. D can often make room for a squeeze-in. This is a clinical decision, not an on-demand booking.

What does the practice handle with pharmacies?

Stock checks, substitution options, prior authorizations moved forward quickly, dose or formulation options when appropriate, and old-school doctor-pharmacist relationships when phone calls move things faster. Pharmacies can fax pharmacy paperwork to (510) 405-6147.

Who is NorCal ADHD for?

California adults who want a doctor who stays involved, can use Spruce for secure messaging, have insurance, and are willing to participate honestly in care.

Who is this not for?

Adults looking for medication-on-demand, anyone under 18, anyone outside California, anyone unwilling to use Spruce, and anyone wanting concierge access without the cost of a private practice.

I had a bad experience with Done or Cerebral. Is this different?

Yes. NorCal ADHD is one doctor and one team, not a venture-backed telehealth product. Dr. D sees every patient himself, stays involved between visits, and runs a small enough practice to remember the details that matter.

Do I need an existing ADHD diagnosis?

No. The first appointment is an evaluation. Dr. D will work with you whether or not you have been diagnosed before.

What if I am not in California?

NorCal ADHD treats California residents only. Telehealth is regulated by where the patient is located, not by where the practice is based.

Is this emergency care?

No. NorCal ADHD is not an emergency service. If you are in crisis or need urgent medical help, call 911 or seek emergency care. Spruce is not monitored for crisis triage.

What kind of doctor is Dr. D?

Dr. D is a primary care physician who has built deep expertise in adult ADHD. He refers out when another setting is the right path.

How are medication decisions made?

Care decisions are clinical and made together with Dr. D. Prior prescriptions are not automatically continued.

What happens at the first appointment?

The first appointment is an evaluation. Dr. D listens, asks real questions, and decides next steps with you.

Do you have a walk-in clinic?

No. NorCal ADHD does not operate walk-in clinics. Care is telehealth-first, and Dr. D can arrange scheduled in-person appointments in select Northern California locations when clinically needed.
